IntroductionThis assignment will be a research regarding the crash of Crossair flight 3597 at Zurich Airport on 24 November 2001, the objective of this assignment is to reflect, list and justify 3 factors contributing to the accident in the context of flight operations. Another goal of this task is to show how we can learn from this incident and at the same time prevent any future incidents that may be similar to this one. Facts about the accident of Crossair Flight 3579 There were several causes of the accident, such as the type of approach, weather conditions, dangerous attitudes and violation of the minimum descent altitude. There was a sudden change in the runway used for landing, due to the new law that closes runway 14, causing the runway to be changed to runway 28. As a result, the pilot had to fly with the very high frequency (VHF) approach. omnidirectional with radio range (VOR) or with distance measuring equipment (DME), since runway 28 did not have an instrument landing system (ILS). The VOR/DME approach is more difficult than the precision approach and the pilot has already shown repeated serious deficiencies in his ability to safely operate an aircraft, particularly the ability to fly a complicated approach and use modern instruments. Therefore, the type of approach is one of the causes that led to the accident. The weather during the time of the accident was poor, with poor visibility and a cloud base. The visual range measured from the airport is above the minimum; however, it was not accurate. The airport's calculation of minimum weather conditions used data from a station distant from Runway 28, causing the meteorological minimums to not reflect actual conditions on Runway 28. Another landed aircraft had warned of poor weather conditions during the run.... .. middle of paper...me. Another minimum time update was provided by one of the flights that landed on runway 28, the pilot of that flight says the minimum time was just at the limit and the runway was only spotted 1.3 miles away. Weather is one of the contributing factors because if the weather minimum had been calculated accurately, the accident would have been avoided as the pilot would have been forced to abort the approach. Conclusion In conclusion, I think that all factors are important and interconnected, since according to the “Swiss Cheese” model one factor alone is not capable of causing the disaster, the disaster will only occur when all the factors coincide. Therefore, all factors are responsible for the crash of Crossair Flight 3597 and must be carefully examined, so as to learn from past mistakes and prevent future similar accidents..
