Unlike reporting near misses, reporting hazards does not require a certain type of event to occur. Hazard warnings usually take the form of a risk assessment matrix or similar diagram to identify inherent risks involved in carrying out particular tasks. An effective risk management system will encourage the use of hazard warnings to give the organization the opportunity to assess risks, identify hazards, and implement controls to mitigate those hazards (Rodrigues & Cusick, 2012).
